Declining libido is one of the most reliable symptoms of testosterone deficiency and one of the most consistently improved by treatment. In men with documented hypogonadism, restoring testosterone to a healthy range typically produces a noticeable improvement in sexual desire within several weeks.
Erectile function is more variable. Testosterone supports the underlying drive and the neural and vascular machinery, but erectile dysfunction is often multifactorial — cardiovascular health, medications, sleep, mental health, and relationship factors all contribute. Some men benefit from testosterone alone; others benefit from a combination of testosterone and a PDE5 inhibitor.
Evaluation always includes cardiovascular risk factors. Erectile dysfunction can be an early warning sign of vascular disease and is worth taking seriously beyond the bedroom.
