Untreated symptomatic hypogonadism rarely 'gets better on its own.' Day-to-day quality of life — energy, mood, libido, body composition — typically continues to decline. Over years, the consequences extend beyond symptoms: accelerated muscle loss (sarcopenia), reduced bone density and fracture risk, worsening insulin sensitivity, and contributions to metabolic syndrome.
Mental health impact is often understated. Men frequently describe a gradual narrowing of life — less engagement with work, less interest in hobbies and relationships, more irritability with family — that they did not connect to hormones until therapy restored what they had lost.
Treatment is not about chasing youth. It is about restoring a baseline that supports the rest of life. That said, therapy is a long-term commitment and is not appropriate for every man — which is why the evaluation matters.
