Cognitive symptoms — word-finding difficulty, slower processing, short-term memory lapses, trouble multitasking — are common during the menopausal transition and in men with low testosterone. Many patients report meaningful improvement in mental clarity once hormones are addressed and sleep is restored.

The mechanisms are biologically plausible. Estrogen supports neurons in the hippocampus and prefrontal cortex. Testosterone influences neurotransmitter systems involved in attention and motivation. Sleep, which often improves with hormone therapy, is itself one of the strongest determinants of cognitive function.

Persistent or progressive cognitive symptoms warrant evaluation beyond hormones. Thyroid disease, B12 deficiency, sleep apnea, mood disorders, and medication effects can all mimic or compound the picture.
